At AIM, reporting a claim is easy. Just follow these instructions and we will expedite your claim as quickly as possible.

• Click on the “Report a Claim” button below and download the PDF form.
• In order to enable the “fillable” PDF form, you will need to open the form using Adobe Acrobat Reader.
• Complete the form electronically or in paper form.
• Submit the form to us via fax, mail or email using the contact information provided in the form.
